Program Entry Requirements

Negative Background Screening
Negative drug test
Complete immunizations
- TB skin test and/or chest X-Ray
- Varicella (2) vaccinations or Titer showing immunity
- MMR (2) or Titer showing immunity
- Tetanus D within last ten years
- Hepatitis B (3) vaccination, positive
- Titer showing immunity or signed waiver
Annual Physician (or designee) Physical with ESCC form completed.
CPR certification at the Health Care Provider Level (American Heart Association)
